To report the death of a member you may fill out the Deceased Member Report Form listed below or contact Boilermakers Local 40 office at (270)769-3022. The earlier we know, the sooner we can begin informing the membership.
Boilermaker Rites
If a deceased member wanted to have Boilermaker Rites performed at their funeral service you must contact one of Boilermakers Local 40 Business Agents during business hours or call (270)274-0011 after hours and weekends. Once again, the earlier we are notified, the sooner we can begin informing the membership. Please have the announcement of Boilermaker Rites included in the obituary if possible.
